Die großen Fortschritte in der Hirnforschung, besonders auch bei den bildgebenden Verfahren zur Erforschung neurophysiologischer Prozesse im Gehirn, haben im neuen Jahrtausend zu der Hoffnung geführt, dass man auch bei den Erkrankungen des Geistes bald sehr viel mehr über deren biologische Ursachen wissen werde. Zu dieser Erwartung beigetragen hat auch die Entzifferung des menschlichen Genoms im Rahmen des »Human Genome Project« (1990–2003). Eine solche verbesserte Kenntnis würde es den Fachleuten ermöglichen – so die Annahme – nicht nur genauere, mit weniger Fehlern behaftete Diagnosen zu stellen, sondern auch neue und zielgerichtetere Therapien entwickeln zu können. Die Aufgabe erwies sich jedoch als viel komplexer als erwartet. Nun soll eine Zwischenstufe zwischen der Erhebung des Phänotyps und jener des Genotyps, sog. Endophänotypen und biologische Marker, wesentlich zur genaueren biologischen Definition geistiger Störungen beitragen.
