Despite the moderate to high risk of venous thromboembolic events in neurosurgical intensive care patients, prophylaxis with heparin has not been established routinely due to the deleterious effects of bleeding complications. After initiating early low molecular weight heparin (LMWH) prophylaxis, we reviewed our patients in order to examine this procedure. The records of patients admitted because of intracranial emergencies were analyzed. Prophylaxis was performed with certoparin (3000 U anti-factor Xa/d, s.c) within 24 hours after admission whenever the control CT did not show a progressive hematoma. Exclusion criteria were coagulation abnormalities (prothrombin time 70%, partial thromboplastin time 40 s, platelet count 100 000/ml, platelet aggregation test sum 60%). The incidence of bleeding complications, deep vein thrombosis (DVT), pulmonary embolism (PE) and resulting morbidity and mortality were assessed. Reasons for admission were traumatic brain injuries (n=344), intracerebral hemorrhage (n=187), subarachnoid hemorrhage (n=88), other reasons (n=25), and a total of 323 patients underwent surgery. Primary contraindications against LMWH were oral anticoagulation (11.0%), antiplatelet agents (7.1%), and thrombopenia (4.0%). Intracranial bleeding complications were recorded in 1.7%, with operative revision in 1.4%, DVT occurred in 0.4%, and PE in 0.2%, all without morbidity or mortality. Heparin-induced thrombopenia was excluded in all patients. In neurosurgical intensive care, early prophylaxis with certoparin was ascertained to be safe and efficacous when contraindications are carefully considered and a 24-hour time interval after admission was guaranteed.
Trotz eines hohen Risikos thromboembolischer Komplikationen wird in der neurochirurgischen Intensivmedizin aus Sorge vor Blutungskomplikationen keine routinemssige Prophylaxe mit Heparin durchgefhrt. In unserer Studie erfolgte eine Thromboseprophylaxe mit dem niedermolekularen Heparin Certoparin (3 000 U anti-factor Xa/d, s.c) innerhalb von 24 Std. nach Aufnahme und Ausschluss einer progredienten Blutung durch ein Kontroll-CT. Ausschlusskriterien waren Gerinnungsstrungen (Quick70%, PTT40 s, Thrombozyten 100 000/ml, Thrombozyten-Aggregationstest 60%). Die Inzidenz von Blutungskomplikationen, tiefen Beinvenenthrombosen, Lungenembolien sowie resultierende Morbiditt/Mortalitt wurden erfasst. Einweisungsdiagnosen waren Schdelhirnverletzungen (n=344), intrakranielle Blutungen (n=187), Subarachnoidalblutungen (n=88), andere Grnde (n=25). Bei 323 Patienten erfolgte ein operativer Eingriff. Primre Kontraindikationen gegen eine Thromboseprophylaxe waren orale Antikoagulation (11%), Thrombozyten-Aggregationshemmer (7,1%) und Thrombopenien (4,0%). Tiefe Beinvenenthrombosen (0,4%), Lungenembolien (0,2%), intrakranielle Blutungskomplikationen (1,8%), mit operativer Revision (1,4%) traten jeweils ohne assoziierte Morbiditt oder Mortalitt auf. Eine Heparin-induzierte Thrombopenie wurde bei allen Patienten ausgeschlossen. In der neurochirugischen Intensivmedizin ist die frhe Thromboseprophylaxe mit Certoparin sicher und zuverlssig, wenn Kontraindikationen beachtet und ein 24-Stunden-Intervall nach Aufnahme vor der ersten Gabe eingehalten werden.
