Hintergrund
Das Konversionssyndrom ist eine funktioneile Störung ohne anatomisch-pathologisches Substrat, bei der eine neurotische Konfliktlösung auf Körperebene erfolgt. Im Gegensatz zur Simulation und Aggravation, einem bewussten Vortäuschen bzw. Übertreiben, entstehen nichtorganische Sehstörungen (herabgesetzte Sehschärfe, Gesichtsfelddefekte, Diplopie u. a.) unbewusst.
Patientin und Methode
8 Monate nach einer Gamma-Knife-Behandlung eines Aderhautmelanoms am rechten Auge wurde eine 56-jährige Patientin aufgrund eines deutlichen Visusabfalls am linken Auge in unserer Ambulanz vorstellig.
Ergebnisse
Der bestkorrigierte Visus links betrug 0,32. Sowohl spaltlampenmikroskopisch als auch funduskopisch konnten keine Pathologien nachgewiesen werden. In der kinetischen und statischen Perimetrie präsentierte sich ein röhrenförmig eingeschränktes Gesichtsfeld. Sämtliche elektrophysiologischen Untersuchungen waren unauffällig.
Diskussion
Aufgrund des unauffälligen neuroophthalmologisch-orthoptischen und elektrophysiologischen Befundes und positiver Tests zur Diagnostik einer psychogenen Störung konnte der Visusabfall links auf eine nichtorganische Erkrankung zurückgeführt werden.
Schlussfolgerung
Die Ursache eines unklaren Visusverlustes, nach Ausschluss einer ophthalmologischen, neuroophthalmologischen oder orthoptischen Pathologie, kann in einzelnen Fällen auch in einer psychogenen, funktionell somatoformen Störung liegen.
