L'objectif de ce travail était de formaliser les principes nécessaires au fonctionnement optimal d'une organisation interprofessionnelle à l'hôpital, en vue de garantir l'adéquation des apports nutritionnels aux besoins spécifiques des patients hospitalisés.L'analyse approfondie des processus a permis de repérer les dysfonctionnements à toutes les étapes, et de formuler des principes (organisation, compétences, environnement, supports, matériels, etc.) nécessaires pour les résoudre et les prévenir.Le premier niveau de dysfonctionnement est l'absence ou la défaillance d'organisation entre les professionnels : définition des responsabilités, organisation spécifique écrite (procédures, etc.), transmissions, protocoles écrits. Il y a également une faible formation des professionnels, particulièrement des médecins, à toutes ces questions. La première partie du référentiel est donc naturellement orientée dans ce sens, avec une première action qui consiste à « obtenir une organisation efficace entre les professionnelset la définition de cinq références visant à prévenir les carences en matière d'organisation. Le second niveau de dysfonctionnement est celui de l'imparfaite définition du contenu des pratiques professionnelles et de l'absence de supports spécifiques, par exemple pour le repérage du risque, la prise en charge diététique et nutritionnelle, le suivi de l'efficacité des actions. La deuxième action proposée dans le référentiel est donc « d'assurer la qualité des pratiques professionnelles autour du patient , en six références.Ce référentiel, en harmonie avec les exigences du manuel Anaes, vient donc s'inscrire en complémentarité et constitue un plan d'objectifs professionnels précis à mettre en œuvre dans les hôpitaux, avec l'aide du CLAN.
The objective of this work was to formalize the principles necessary for the optimal operation of an interprofessional organization at the hospital, in order to provide nutritional intakes according to the specific requirements of inpatients.The analysis of the processes made it possible to identify malfunctions and to formulate principles (organization, competence, environment, supports, hardware, etc.) necessary to solve and prevent them.The first level of abnormal operation is the absence or the failure of organization between professionals: the definition of responsibilities, not having a written specific organization (for example, procedures), transmissions, written protocols. Training of the professionals, particularly of medical doctors, needs to be improved for all of these questions. The first action of the reference frame consists thus in ‘obtaining an effective organization between the professionals’ with the definition of five references aimed at preventing deficiencies in terms of organization.The second level of abnormal operation is the imperfect definition of the contents of professional practices, and the absence of specific supports, for example, inidentifing nutritional risk, the nutritional care, and the follow-up of the effectiveness of the actions. The second action in the reference frame is thus to ‘ensure the quality of the professional practices around the patient’, and includes six references.This reference frame, which complies with the requirements of the ANAES handbook, constitutes a plan of professional objectives to implement in hospitals, with the assistance of the CLAN.
