Hintergrund – Das kutane T‐Zell Lymphom (CTCL) bei Hunden ist ein heterogener Krankheitskomplex, welcher aus nicht‐epitheliotropen (NE) und epitheliotropen Formen besteht. Diese Lymphome können durch die Anwesenheit von dominanten Populationen von zytologisch atypischen Lymphozyten leicht erkannt werden.
Ziel – Das Ziel dieser Studie war es, die Schlüsselmerkmale von entzündlichen NE‐CTCL vorzustellen, welche leicht mit einer reaktiven, entzündlichen histiozytären Erkrankung verwechselt werden können.
Tiere – Vierundzwanzig Hunde (Durchschnittsalter 7,5 Jahre) wurden mit entzündlichem NE‐CTCL vorgestellt. Die Läsionen bestanden aus Knoten, Plaques oder größeren Umfangsvermehrungen. Eine erste Diagnose von kutaner reaktiver Histiozytose (11 Hunde) oder histiozytären Neoplasien (drei Hunde) wurden von den erstbeurteilenden Pathologen gestellt.
Methoden – Die Veränderungen wurden histologisch und immunhistochemisch beurteilt, um canine Leukozytenantigene zu finden. Genomische DNA wurde aus den Hautveränderungen extrahiert und eine Analyse des Gen Rearrangements des T‐Zell Rezeptor Υ Lokus durchgeführt.
Ergebnisse – Die kutanen Läsionen bestanden aus pleozellulärer Infiltration der Dermis mit unterschiedlicher Ausdehnung in die Subkutis. Die Veränderungen bestanden oft um Blutgefäße herum und im Bereich von Hautanhangsgebilden. Ein Epitheliotropismus war minimal oder fehlte zur Gänze. Kleine Lymphozyten, Plasmazellen und mittelgroße bis große, zytologisch atypische Lymphozyten waren zwischen den prominenten histiozytären Infiltraten verteilt. Atypische Lymphozyten zeigten oft eine deutliche Variation im Ausmaß ihrer CD3 Exprimierung. Molekulare Klonalitätsanalyse des T‐Zell Rezeptor Υ Lokus zeigte eine klonale Ausbreitung der T Zellen bei 22 der 23 getesteten Hunde.
Schlussfolgerung – Das Erkennen eines entzündlichen NE‐CTCL und seine Differenzierung von einer kutanen reaktiven Histiozytose hängen von einer genauen Beurteilung der Morphologie der Lymphozyten und ihrer Immunfärbung ab. Eine Bestätigung der Diagnose wird am besten durch eine Analyse des T‐Zell Rezeptorgen Rearrangements erzielt.
