Aunque pocas publicaciones hablan sobre el manejo clínico de pacientes con síntomas de rabia en unidades de cuidados intensivos, la gran mayoría de casos de rabia humana ocurren en emplazamientos rurales de países en vías de desarrollo, en donde los trabajadores sanitarios son pocos, carecen de formación y de medicamentos. Basándonos en nuestra experiencia, sugerimos como los clínicos trabajando en lugares con recursos limitados pueden hacer un mejor uso de los medicamentos esenciales a la hora de ofrecer asistencia a pacientes con rabia y a sus familias, sin ponerse ellos en riesgo. Un manejo comprensivo y compasivo de los pacientes con rabia furiosa debería tener como objetivo aliviar la sed, la ansiedad y los ataques epilépticos utilizando infusiones, diazepam o midazolam y medicamentos antipiréticos por via intravenosa o rutas intrarrectales. Aunque el paciente esté muriendo, se debe evitar el fallo respiratorio, especialmente si la familia, después de haber sido informada, quiere llevarse al paciente a casa con vida, para seguir los ritos funerarios. El personal sanitario debería recibir entrenamiento y las guías clínicas deberían actualizarse para incluir cuidados paliativos para la rabia en países endémicos.
