Contexte
Le paludisme provoque des réponses inflammatoires qui, sans régulation adéquate, peuvent exercer des effets néfastes. Une fois activé, le récepteur de déclenchement 1 exprimé sur les cellules myéloïdes (TREM‐1) accentue les réponses inflammatoires en augmentant la sécrétion d’IL‐8 et d'autres cytokines Th1. En revanche, des transcrits similaires à TREM‐1 (TREML‐1) favorisent les réponses anti‐inflammatoires en se liant aux ligands TREM‐1 et en compétition avec TREM‐1, antagonisant ainsi l'activation de TREM‐1 pour réduire l'inflammation. Le récepteur endothélial de la protéine C (EPCR) médie également les réponses anti‐inflammatoires en activant la protéine C endothéliale (PC). Lors de la stimulation microbienne, des formes solubles de TREM‐1 (sTREM‐1) et d’EPCR (sEPCR) sont libérées. Leurs concentrations plasmatiques reflètent le degré d'inflammation et la sévérité de l'infection.
Méthodes
Dans une étude transversale comparant les patients atteints de paludisme sévère à ceux avec un paludisme non compliqué, les taux plasmatiques de sTREM‐1, TREML‐1 soluble (sTREML‐1) et sEPCR ainsi que les taux plasmatiques de sEPCR provenant de patients convalescents ont été quantifiés. Les échantillons ont été prélevés à l'admission des patients pédiatriques infectés par Plasmodium falciparum dans les hôpitaux d'Accra, au Ghana. Les régions génétiques distinctes des gènes codant pour TREM‐1, EPCR, interleukine (IL)‐8 et IL‐18 comportant des polymorphismes génétiques connues qui influencent les taux plasmatiques ont subi un séquençage d'ADN.
Résultats
Des taux élevés de sTREM‐1 ont été observés chez les enfants atteints de paludisme sévère par rapport à ceux avec un paludisme non compliqué (P = 0,049). De faibles rapports TREM‐1/TREML‐1 ont été associés au paludisme non compliqué (P = 0,033). La variante TREM1 rs2234237T provoquant un changement d'acides aminés Thr25Ser, qui a été associé à des taux plasmatiques plus élevés de TREM‐1, était significativement plus fréquente chez les patients atteints de paludisme sévère que chez ceux atteints de paludisme non compliqué (P = 0,036). De faibles taux de sEPCR ont été observés dans le paludisme sévère et non compliqué, alors que les variantes génotypiques de IL‐8, IL‐18 et EPCR ne montrent aucune association.
Conclusion
Des taux plasmatiques plus élevés de sTREM‐1 seul ou relatifs à sTREML‐1 au cours du paludisme, prédisposent au phénotype de paludisme sévère. Le portage de l'allèle TREM1 rs2234237T semble être un facteur de risque pour le développement du paludisme sévère.
