Abstract: Live donor renal transplantation remains the best treatment option for end stage renal failure in pediatric patients (1–3). Better understanding of the hemodynamics of donor–recipient size discrepancy and advances in interventional techniques with improved surgical techniques have decreased the incidence and severity of surgical complications and enhanced graft survival (1, 2). We describe a rare complication occurring intra‐operatively in a pediatric renal transplant resulting in acute limb ischemia and the surgical option taken.
