We describe a case of an 18‐year‐old man presenting with syncope found to have short‐coupled premature ventricular complexes (PVCs) with subsequent nonsustained polymorphous ventricular tachycardia (PVT). Electrophysiology testing revealed premature PVCs and PVT provoked by calcium but not isoproterenol. It was noted that the earliest triggered event appeared to arise from ventricular muscle with subsequent involvement of the fascicles and these areas were ablated.
