Indicadores reportados por los padres para detectar dificultades de alimentación y deglución y desnutrición en niños de edad pre‐escolar con parálisis cerebral
Objetivo
Determinar los indicadores más exactos reportados por los padres para detectar (1) dificultades de alimentación/ deglución y (2) desnutrición en niños de edad ´pre‐escolar con parálisis cerebral (PC)
Método
Este fue un estudio longitudinal, considerando 179 niños con PC, entre 18 y 60 meses de edad (promedio 34.1 meses [ DS 11.9] al inicio, 111 varones, 68 mujeres [Sistema de Clasificación de Función Motora Gruesa (GMFCS) nivel I: 84, II:23; III: 28; IV:18; V: 26] en total 423 datos).
Las dificultades de succión/alimentación se determinaron con la Dysphagia Disorders Survey (Encuesta sobre los Trastornos Disfágicos) y 16 signos sugerentes de trastorno de fase faríngea. La desnutrición se consideró por puntaje Z compuesto por peso/talla y pliegue cutáneo menor que z=‐2. Los indicadores primarios reportados por los padres incluyeron la duración de la comida, el estrés en la hora de comer, la preocupación por el crecimiento y los problemas respiratorios. Otros indicadores se derivaron de un cuestionario de alimentación de los padres, incluyendo “dificultad significativa para comer / beber”. Los datos se analizaron mediante la regresión de efectos mixtos multinivel y las estadísticas de diagnóstico.
Resultados
Los indicadores primarios reportados por los padres asociados con la alimentación / deglución fueron “estrés moderado‐severo para los padres” (razón de momios [RM] = 3,2 [intervalo de confianza del 95% {IC} 1,3‐7,8]; p <0,01); P <0,01), y “cualquier condición respiratoria” (RM = 1,8 [IC del 95%: 1,4‐5,8], p <0,01). El indicador asociado con la desnutrición fue “preocupación moderada‐grave con respecto al crecimiento” (peso en altura RM= 13,5 [IC del 95%: 3,0‐61,3], p <0,01, pliegue RM = 19,1 [IC del 95%: 3,7‐98,9] p <0,01 ). “Sensibilidad = 58,6%, especificidad = 100,0%) y” preocupación de los padres con respecto al crecimiento” por la desnutrición (sensibilidad = 77,8%, especificidad = 77,0%).
Interpretación
Los indicadores reportados por los padres son factibles para detectar las dificultades de la alimentación/deglución y la desnutrición, en los niños con PC, pero necesitan una validación formal.
