Diseño de un Sistema de Apoyo para la Toma de Decisiones a Tiempo Real Para las Resucitaciones Traumatológicas
Introducción
El uso de un sistema electrónico de apoyo para la toma de decisiones clínicas (SEDC) se ha recomendado para mejorar la implementación de las reglas de decisión clínica. Sin embargo, muchos SEDC están diseñados e implementados sin tener en cuenta el contexto en el cual se desarrolla la labor asistencial. La implementación de la regla de decisión clínica para la lesión cerebral traumática (LCT) pediátrica en un servicio de urgencias (SU) pediátrico de nivel 1 incluye un cuestionario electrónico cuando se solicita una TC craneal mediante una entrada de órdenes médicas computarizada (EOMC). Los sanitarios usan esta herramienta de EOMC en menos de un 20% de las causas de resucitación en traumatología. Una consideración técnica de los factores humanos podría identificar las barreras en la implementación que limitan el uso de esta herramienta.
Objetivos
El objetivo fue diseñar un SEDC en la LCT pediátrica para la resucitación en traumatología mediante la consideración de los factores humanos. La hipótesis fue que los expertos clínicos clasificarán mejor con un SEDC que potencie su uso que con el SEDC existente debido al diseño de interfaz de usuario y a la idoneidad para el uso clínico.
Metodología
Este estudio de metodología mixta cumplió los principios de evaluación de facilidad de uso. Los urgenciólogos pediátricos fueron encuestados para identificar las barreras en el uso del SEDC existente. Mediante el uso de los protocolos de resucitación de traumatología habituales, se desarrolló un análisis de tareas jerárquicas de la evaluación de la LCT pediátrica. Cinco expertos clínicos, todos miembros universitarios acreditados en medicina de urgencias y emergencias pediátricas, modificaron después de forma iterativa el análisis de tareas jerárquicas hasta alcanzar un consenso. El equipo informático desarrolló una pantalla prototipo de SEDC usando un análisis de tareas jerárquico. Tres técnicos en factores humanos proporcionaron retroalimentación en el prototipo a través de la evaluación heurística, y el equipo informático refinó el SEDC usando un proceso de hacer prototipos rápido. El SEDC se sometió después a una evaluación de facilidad de uso iterativa por los cinco expertos clínicos usando una revisión de vídeo de 50 casos de resucitación en traumatología. Se creó un SEDC basándose en sus retroalimentaciones, con el análisis de contenido de las evaluaciones realizadas para asegurar que todas las inquietudes se identificasen y valorasen.
Resultados
Entre los 26 urgenciólogos (76% de porcentaje de respuesta), las principales barreras para el uso de la herramienta existente fueron que la información visualizada es redundante y no se adapta al flujo de trabajo clínico. Después que el nuevo prototipo de SEDC se desarrollara basándose en el análisis de tareas jerarquizadas en la resucitación de traumatología, los técnicos de factores humanos la consideraron mejor que el SEDC previo para 9 de cada 10 usuarios habituales en el diseño de interfaces heurísticas en una escala de 3 puntos. El nuevo SEDC también se consideró mejor para el uso clínico, mediante la misma escala, en un 84% de los 50 pares de vídeos de expertos y se clasificó equivalente en el resto de casos. Los expertos clínicos también consideraron barreras para el uso del nuevo SEDC, aunque fueron bajas.
Conclusiones
El diseño de un SEDC para imágenes diagnósticas usando una metodología técnica de factores humanos ha mejorado la percepción de utilización entre los urgenciólogos pediátricos.
