Hintergrund..
The CRASH-syndrome is a group of X-linked diseases showing a mutation in the L1CAM-gene. According to the acronym CRASH the main symptoms are corpus callosum hypoplasia, mental retardation, adducted thumbs, spastic paraplegia and hydrocephalus. A disease out of this group is the X-linked hydrocephalus, first described by Brickers and Adams 1949 as a hydrocephalus with stenosis of the aqueduct of Silvius (HSAS). This case report describes a four months old child with massive internal hydrocephalus and adducted thumbs. We could find a mutation in the L1CAM-gene which is located near the long arm of the X chromosome in Xq28. It encodes the neuronal cell adhesion molecule, needed for axonal spreading. The intra- and interfamilial variability in families with an L1 mutation is very wide.
