Einleitung. Osteoporose ist die hufigste Knochenerkrankung. Es ist abzusehen, dass durch zunehmende beralterung der Bevlkerung auch die Bedeutung der Osteoporose und ihrer Behandlung einen steigenden Stellenwert einnehmen wird. Hintergrund. Schmerzhafte osteoporotische Wirbelkrperfrakturen fhren zu einer signifikant erhhten Morbiditt und Mortalitt. Die Standardbehandlung des osteoporotisch induzierten Wirbelkrpereinbruchs umfasst Schmerztherapie, Schonung und im Bedarfsfall orthetische Versorgung. Methode. Mithilfe der reinen Zementaugmentierung kann eine gute Schmerzreduzierung und damit auch eine Verbesserung der Lebensqualitt erzielt werden. Mit der Technik der Kyphoplastik kann zustzlich die durch die Fraktur entstandene Deformitt korrigiert werden. Nach Punktion des Wirbels wird durch eine Fhrungshlse ein Ballon eingebracht und aufgeblasen. Die so entstandene Kavitt wird nach Entfernen des Ballons mit Knochenzement (Polymethylmetacrylat, PMMA) aufgefllt. Diskussion. Mit der Kyphoplastik knnen osteoporotisch bedingte kyphotische Deformitten korrigiert werden. Die risikormere Kyphoplastik ist im Vergleich zur Vertebroplastik die zeit- und kostenaufwndigere Methode. Es wird sich zeigen, ob der erwartete Vorteil durch die Korrektur der kyphotischen Deformitt lngerfristig fr den Patienten die besseren Ergebnisse, wie etwa eine geringere Refrakturrate, erbringen wird.
Introduction. Osteoporosis is the most common bone disease. Due to an increase of the older population an higher impact of osteoporosis and its treatment can be expected. Background. Painful osteoporotic vertebral fractures result in an increased morbidity and mortality. Standard treatment of painful osteoporotic vertebral fractures comprises analgetics, bed rest and, if needed, orthotics. Method. By mere augmentation of the vertebra with polymethylmet acrylate (PMMA), the so called vertebroplasty a good pain reduction and increase in function and quality of life can be achieved. With the technique of kyphoplasty (Kyphon) it is feasable to correct a kyphotic deformity. Inflatable ballons are introduced to the vertebra to lift the endplates. The created cavity is filled with bone cement (PMMA). Discussion. Kyphoplasty is able to correct osteoporosis induced kyphotic deformity. Compared to the technique of vertebroplasty, kyphoplasty is less riskful but is more time consuming and more expensive. Time will show whether the expected advantage of deformity correction will result in a better outcome for the patient, e.g. in a lower incidence of refractures.
