In den letzten Jahren ist eine Zunahme von opportunistischen Pilzinfektionen bei Patienten mit hmatoonkologischen Erkrankungen registriert worden. Eine zentrale Rolle spielen Infektionen durch Hefepilze wie Candida albicans und sog. Candida-non-albicans-Hefen, insbesondere jedoch durch Fadenpilze wie Aspergillus fumigatus und andere Aspergillus-Arten. Die Aspergillose stellt ein multifaktorielles Problem dar. Es muss davon ausgegangen werden, dass diese Pilzinfektionen nie vollstndig vermieden werden knnen, da Aspergillen ubiquitr vorkommen. Viele Erfahrungen zeigen jedoch, dass das Risiko einer Aspergillus-Infektion durch optimale Prventionsmanahmen gemindert werden kann. Schutz prdisponierter Patienten vor einer Exposition gegenber Pilzsporen, die Reduktion von Infektionsquellen und die frhzeitige Erfassung einer Kolonisation sind Teil des Managements in der Infektionsprvention. Als besondere Streuquellen gelten Baumanahmen im Krankenhaus. Voraussetzung fr die zielgerichtete Etablierung von Schutzmanahmen ist die genaue Kenntnis der Risikogruppen und der wichtigsten Risikofaktoren. Die Umsetzung spezieller Isoliereinrichtungen (high-efficiency air filtration system) zum Schutz der Patienten sollte Voraussetzung fr die Behandlung von Risikopatienten sein. Die umzusetzenden Manahmen sollen die Prvention und frhzeitige Erkennung von Infektionen entsprechend 1 des Infektionsschutzgesetzes untersttzen.
Fungal pathogens are recognized as a major and increasing source of life-threatening infections in the immunocompromised host. Infections due to Aspergillus species are among the most common causes of nosocomial pneumonia in patients with hematological malignancies and are associated with an extremely high mortality rate. Aspergillus infections are often related to new building construction and reconstruction, roadwork, contaminated air conditioning systems,and contaminated potting soils. Endogenic sources include asymptomatic colonization of the respiratory tract and pulmonary aspergilloma. Suitable measures in the patients environment to protect him from exposure to fungal spores may result in a marked reduction of infection risk. The use of high-efficiency particulate air (HEPA) filtration systems during hospitalization represents the current standard in prevention of invasive pulmonary aspergillosis. This article addresses epidemiological aspects, exposure factors, and preventive aspects with special attention to construction work in hospitals.
