Die Behandlung von Defektpseudarthrosen zählt zu den schwierigsten Aufgaben der modernen Traumatologie. Mehrere Probleme müssen gleichzeitig bzw. in sinnvoller Abfolge therapiert werden. Die Infektion, die Instabilität des Knochens und der sekundäre Knochenaufbau bedeuten immer mehrere Operationen. Die individuelle Compliance des Patienten muss Eingang in die strategischen Überlegungen finden. Nur mit allen modernen Verfahren der Rekonstruktion vertraute und diese auch rund um die Uhr vorhaltende Kliniken sollten dieses Problem behandeln. Auch wenn die Erfolge in darauf spezialisierten Zentren als extrem hoch anzusehen sind, bleiben immer Dauerschäden für den Patienten zurück. Oberstes Prinzip ist, diese zu vermeiden, was nicht immer gelingen wird, aber zumindest angestrebtes Ziel sein muss. Hier ist in der Zukunft eine bessere Steuerung der Heilverfahren von den Kostenträgern mit frühzeitiger Vorstellung in Kompetenzzentren zu fordern, um den Patienten langfristige Heilverläufe mit Defektheilung zu ersparen. Der zunehmende Kostendruck könnte eine Chance sein, allerdings nur, wenn die entsprechenden Kompetenzzentren auch eine korrekte Vergütung erhalten.
