Raucher entwickeln 2- bis 3-mal hufiger eine periphere arterielle Verschluss-Krankheit (AVK) als Nichtraucher. AVK-Patienten, die den Nikotinabusus fortsetzen, haben eine ausgeprgte Progression der Atherothrombose. Die Gehstrecke bei Claudicatiopatienten und die prospektiven Amputationsraten korrelieren direkt mit der Fortsetzung des Nikotinabusus. Ein vernnftiges Gehtraining sollte behutsam aber regelmig (etwa 3-mal pro Woche) und am besten in Claudicatiogruppen erfolgen. Die bei der Behandlung der AVK eingesetzten Medikamente haben insgesamt das Ziel, das stadientypische Krankheitsbild der chronischen Verschlusskrankheit positiv zu beeinflussen. Kommen zur peripheren arteriellen Verschlusskrankheit als weitere Risikofaktoren Diabetes mellitus, Fettstoffwechselstrungen oder zu hoher Blutdruck hinzu, mssen diese Begleiterkrankungen konsequent medikaments behandelt werden. Dazu eignet sich eine Reihe von Medikamenten, deren Wirksamkeit durch wissenschaftliche Studien belegt ist. Bei einigen alternativen Behandlungsmethoden ist dagegen Vorsicht geboten.
Smokers develop peripheral arterial occlusive disease (PAOD) two to three times more frequently than nonsmokers. PAOD patients who continue their nicotine abuse exhibit a pronounced progression to atherothrombosis. The walking distance in patients with claudication and the prospective amputation rates directly correlate with the continuation of nicotine abuse. Prudent walking training should be instituted cautiously but regularly (about three times per week) and is best in groups of claudication patients (supervised classes). Medications administered for the treatment of PAOD all are intended to exert a positive influence on the clinical picture typical for the stage of chronic occlusive disease. If peripheral PAOD is accompanied by further risk factors such as diabetes mellitus, disorders of lipometabolism, or elevated blood pressure, the concomitant diseases must be treated by medication. Several medications with scientifically proven efficacy are appropriate. In contrast, caution is advisable when considering some alternative treatment methods.
