Eine wesentliche Funktion neurologischer Intensivstationen stellt die Überwachung schwerkranker Patienten dar. Der ausgedehnte Schlaganfall ist eine Erkrankung mit hoher Mortalität. Eine klinische Beurteilung dieser Patienten ist aufgrund der meist bestehenden Analgosedation oder des Komas schwierig, weshalb ein zusätzliches technisches Monitoring notwendig ist. Es erleichtert den behandelnden Ärzten die Einschätzung des klinischen Zustandes. Außerdem lassen sich pathophysiologische Veränderungen so frühzeitig erkennen und therapeutisch angehen.
Bei dem bisher eingesetzten Basismonitoring werden nichtinvasive und invasive Methoden eingesetzt. Zusätzlich wird z. Z. in einigen Zentren das multimodale Monitoring getestet. Es erlaubt eine kontinuierliche Überwachung physiologischer Parameter der Hirnfunktion. Dadurch lassen sich frühzeitig bisher erst in einem Spätstadium der Erkrankung erkennbare Veränderungen diagnostizieren und therapeutisch angehen. Das multimodale Monitoring kann somit eine sinnvolle Ergänzung der intensivmedizinischen Überwachung darstellen.
Der vorliegende Artikel möchte Überwachungsmethoden der neurologischen Intensivstation und deren Bedeutung für die Schlaganfallbehandlung erläutern und neue Entwicklungen des invasiven Monitorings darstellen.
