Hintergrund: In der Therapie des kolorektalen Karzinoms wurden in den letzten Jahren bedeutende Fortschritte erzielt, von denen viele bei der Jahrestagung der American Society of Clinical Oncology (ASCO) im Mai 2005 in Orlando, FL, USA, vorgestellt wurden.
Methodik: In dieser Übersichtsarbeit werden die Höhepunkte der Vorträge und Posterpräsentationen des ASCO-Kongresses 2005 sowie die wichtigsten neuesten Veröffentlichungen beschrieben.
Ergebnisse und Fazit: Therapie des metastasierten kolorektalen Karzinoms: Studien konnten zeigen, dass Capecitabin 5-Fluorouracil (5-FU) in der Kombination mit Oxaliplatin möglicherweise ersetzen kann. Der EGF-Rezeptor-Antikörper Cetuximab zeigt Wirksamkeit als Monotherapeutikum und in Kombination mit Irinotecan, Studien untersuchen die Kombination mit Oxaliplatin/5-FU. Der VEGF-Antikörper Bevacizumab führt zu einer Verbesserung der Wirksamkeit von allen bislang untersuchten Chemotherapien, ist als Monotherapeutikum jedoch weitgehend unwirksam.
Adjuvante Therapie des Kolonkarzinoms: Eine Kombination aus Oxaliplatin und 5-FU verbessert das krankheitsfreie Überleben im Vergleich mit 5-FU-Regimen und ist als neuer Goldstandard für die adjuvante Therapie des Stadiums III anzusehen. Capecitabin zeigt bei signifikant besserer Verträglichkeit eine mindestens gleiche Wirksamkeit wie 5-FU-Bolusregime und sollte diese, bei Verzicht auf Oxaliplatin, ersetzen. Irinotecan führt in der adjuvanten Therapie zu keiner Verbesserung des krankheitsfreien Überlebens und ist daher ohne bewiesenen Stellenwert. Monoklonale Antikörper werden in Studien in der adjuvanten Therapie untersucht.
