Schmerzen des Bewegungsapparats sind ein relativ häufiger Vorstellungsgrund in der Pädiatrie. Dem Knochenschmerz können einerseits maligne Erkrankungen, wie primäre Knochentumoren, Knochenmetastasen und Knochenmarkerkrankungen, oder andererseits entzündliche Prozesse, wie bakterielle Osteomyelitiden und nichtbakterielle Osteitiden zugrunde liegen. Als weitere Differenzialdiagnosen sind Schmerzverstärkungssyndrome und Wachstumsschmerzen zu nennen. Selten liegen endokrinologische Prozesse zugrunde. Dieses breite Spektrum an Erkrankungen erschwert die Diagnosestellung und die Auswahl einer angemessenen Diagnostik. Gerade bei Malignomen und bakteriellen Osteomyelitiden sind die zügige Diagnosestellung und Einleitung einer adäquaten Therapie jedoch unerlässlich und prognostisch entscheidend. Schmerzverstärkungssyndrome werden v. a. bei Patientinnen im Jugendalter beobachtet.
