Resumen
Objetivos. Comparar el coste y las consecuencias sanitarias de tres pruebas diagnósticas de la rotura prematura de membranas (RPM) no confirmada clínicamente: AmniSure, las pruebas habituales (medida del pH con nitrazina y/o cristalización en helecho del líquido amniótico) y ActimProm.
Métodos. Modelo farmacoeconómico de análisis de decisiones, para las siguientes variables: sensibilidad y especificidad de las pruebas diagnósticas, prevalencia de la RPM, costes sanitarios y probabilidad de aparición de complicaciones a consecuencia de los diagnósticos falsos negativos (muertes perinatales, colonización materna y del recién nacido por estreptococos del grupo B -EGB- u otras bacterias, meningitis y secuelas de la meningitis). Se analizó un caso básico, considerando los valores medios de las variables y se efectuaron análisis de sensibilidad simples univariantes.
Resultados. La probabilidad de obtener resultados falsos positivos y falsos negativos es mucho menor con AmniSure (1,2% y 5,9%) que con las pruebas habituales (pH con nitrazina: 16,3% y 34,2%; cristalización en helecho: 27,1% y 25,6%) y con ActimProm (13,0% y 8,8%). El coste por cada paciente diagnosticada mediante AmniSure, las pruebas habituales y ActimProm sería de 357 €, 598 € y 377 €, respectivamente, con menos de 26 semanas de gestación, de 378 €, 619 € y 398 €, respectivamente, con 262–34 semanas de gestación y, finalmente, de 336 €, 577 € y 357 €, respectivamente, con 35 o más semanas de gestación. Por tanto, el ahorro por cada paciente diagnosticada con AmniSure sería de 241 € en comparación con las pruebas habituales, y de 20,27 € en comparación con ActimProm. Los análisis de sensibilidad confirmaron la estabilidad del caso básico en la mayoría de los supuestos considerados, con ahorros por paciente diagnosticada con AmniSure que oscilaron entre 18 € y 608 € frente a las pruebas habituales y entre 2 € y 136 €, en comparación con ActimProm, con la única excepción del supuesto con los valores máximos de sensibilidad-especificidad, en cuyo caso ActimProm generaría un ahorro de 11 €. La menor probabilidad de resultados falsos negativos con AmniSure daría lugar a una reducción en el número de complicaciones en comparación con las pruebas habituales, evitándose aproximadamente 3 muertes perinatales, 10 casos de meningitis por EGB y 2 secuelas (leves y/o moderadas) de la meningitis, por cada 1.000 diagnósticos realizados. Frente a ActimProm se evitaría 1 caso de meningitis por EGB.
Conclusiones. De acuerdo con el modelo, la mayor fiabilidad del diagnóstico de la rotura prematura de membranas pretérmino con AmniSure reduce considerablemente las complicaciones fetales y neonatales, con un menor coste por paciente.
