Hintergrund
Das Gesamtüberleben von Patienten mit fortgeschrittenem Ovarialkarzinom nach optimaler zytoreduktiver Chirurgie und adjuvanter Chemotherapie konnte in den letzten 4 Jahrzenten verbessert werden, liegt aber trotzdem bei 35–45 %. Somit bleibt das Ovarialkarzinom das tödlichste aller Genitalkarzinome. Großer Bedarf besteht in der Optimierung der Effektivität des aktuellen Therapiestandards aus Carboplatin und Paclitaxel sowie der Reduktion von Akut- und Langzeitnebenwirkungen.

Evidenzbasierte Zusammenfassung der aktuellen Therapieempfehlungen sowie neuer Behandlungsoptionen.
Material und Methode
Nach systematischer Literaturrecherche wurden relevante Publikationen, Kongressberichte sowie im Rahmen derzeit laufender Studien getestete neue Therapiestrategien zusammengefasst.
Ergebnisse
Für das Management des frühen Ovarialkarzinoms ist ein adäquates, intraoperatives Staging essenziell. Außer beim FIGO Ia, G1 sollte eine Chemotherapie erwogen und individuell diskutiert werden. Beim fortgeschrittenen Ovarialkarzinom stellt die Hemmung der Angiogenese eine neue hoch effektive Therapieoption dar. Seit Ende 2011 ist Bevacizumab für die adjuvante Therapie der fortgeschrittenen Stadien FIGO IIIb–IV in Kombination mit Carboplatin/Paclitaxel zugelassen.
Schlussfolgerungen
Langfristiges Ziel im Management des Ovarialkarzinoms ist die Implementierung zielgerichteter Substanzen, welche unter Reduktion von Nebenwirkungen bei gleichbleibend gutem Therapieerfolg eine Optimierung des therapeutischen Index sichern können. Laufende Studien fokussieren sich auf weitere Möglichkeiten der Angiogenesehemmung. Die Herausforderung für die Wissenschaftler bleibt die Identifizierung von Subgruppen von Patientinnen, welche von den unterschiedlichen Behandlungsstrategien am besten profitieren könnten.
