GRUNDLAGEN: Am Beispiel derzeit verwendeter Leitlinienmodelle wird die Komplexität eines Prozesses der Leitlinienentwicklung dargestellt. METHODIK: Literaturübersicht über Leitlinienmodelle. ERGEBNISSE: Leitlinien sind prinzipiell ein nützliches Instrument, um die Qualität medizinischer Abläufe zu verbessern und den Patienten gleichzeitig eine Entscheidungshilfe für seine Behandlung zu geben. Trotz vieler Versuche Praxisleitlinien routinemäßig in medizinische Algorithmen zu implementieren, sind nur wenige verwertbare CPGs verfügbar. Wenn die Leitlinien jedoch nur als Mittel zur Kostenreduktion für einzelne partikularistische Gruppen entwickelt werden und noch dazu starr und unflexibel gestaltet sind, so können sie sogar den gegenteiligen Effekt bewirken. SCHLUSSFOLGERUNGEN: Leitlinien sollten also eher ein Bestandteil eines allumfassenden Qualitätsbedürfnisses der beteiligten Gruppen sein, als ein von außen verordnetes, starres Korsett mit dem Geruch der Verordnung.
