Resumen Objetivo
Realizar un análisis de coste-efectividad de apixabán frente a rivaroxabán en la prevención del ictus en pacientes con fibrilación auricular no valvular (FANV) en España.
Métodos
Modelo de Markov, con ciclos de 6 semanas, durante toda la vida del paciente y 10 estados de salud como ictus, sangrados y otras complicaciones cardiovasculares. La eficacia y seguridad de los fármacos se obtuvo de un metanálisis de comparaciones indirectas por parejas. El análisis se hizo desde la perspectiva del Sistema Nacional de Salud (SNS) y de la sociedad. El coste de los medicamentos se calculó según las dosis recomendadas (apixabán: 5 mg dos veces al día; rivaroxabán: 20 mg/día). El coste de las complicaciones de la FANV y del manejo de la enfermedad se obtuvo de fuentes españolas. Se aplicó una tasa de descuento del 3,5% anual para costes y resultados sanitarios.
Resultados
Los resultados indican que, en una cohorte de 1.000 pacientes con FANV, con apixabán se evitarían numerosas complicaciones (9 ictus, 10 sangrados intracraneales, un embolismo sistémico, 32 sangrados mayores, 52 sangrados menores clínicamente relevantes y 12 muertes relacionadas) en comparación con rivaroxabán. En consecuencia, en cada paciente tratado con apixabán se obtendrían más años de vida (0,062 AVG) y más años de vida ajustados por calidad (0,049 AVAC ganados). Con apixabán se generarían más costes por paciente que con rivaroxabán para el SNS (115 €) pero se producirían ahorros desde la perspectiva social (−241 €), con un coste por AVG y AVAC ganado de 1.855 € y 2.347 € para el SNS, siendo apixabán dominante (más eficaz y con menos costes que rivaroxabán) desde la perspectiva de la sociedad. Los análisis de sensibilidad determinísticos y probabilísticos confirmaron la estabilidad del caso base para ambas perspectivas.
Conclusiones
Según lo indicado por el presente estudio, se puede concluir que apixabán es un tratamiento coste-efectivo en comparación con rivaroxabán en la prevención del ictus en pacientes con FANV.
