L’échec d’extubation induit des morbidités et une augmentation de la mortalité. Le kinésithérapeute peut intervenir afin de faciliter le succès de ce geste. Matériel et méthode : Une recherche bibliographique a été réalisée de 2004 à 2014 grâce aux bases de données Medline, PEDro et Cochrane Library.

Résultats
Onze revues de littérature, une méta-analyse, six études contrôlées randomisées, dix études prospectives, trois recommandations d’experts ont été analysées. Les techniques validées sont : l’utilisation d’un protocole de sevrage, la réalisation du test de fuite, le traitement et la prévention des atélectasies, la mobilisation précoce et la ventilation non invasive (grade A); le désencombrement manuel du patient intubé et ventilé (grade B), le désencombrement manuel du patient extubé (grade B) et l’oxygénation à haut débit (grade B); l’évaluation de la force de toux et de l’encombrement (grade C), la mesure du P0,1/PiMax (grade C), le handgrip strength test (grade C). Les techniques invalidées sont : le renforcement des muscles inspirateurs (grade A), l’évaluation de la déglutition et le désencombrement instrumental (grade C).
Conclusion
Le kinésithérapeute peut jouer un rôle essentiel dans l’extubation. Cependant, plusieurs techniques nécessitent une validation scientifique plus importante.
