Die Behandlung maligner Tumoren der Gesichtshaut ist ein wichtiger Bestandteil des Fachs Mund-, Kiefer- und Gesichtschirurgie. In den kommenden beiden Fortbildungsbeiträgen werden für den Gesichtsbereich wichtige Aspekte häufiger Entitäten behandelt. Der vorliegende erste Beitrag beschäftigt sich mit dem Basalzellkarzinom, dessen mit Abstand häufigste Lokalisation der Gesichtsbereich ist. Die Behandlungsmodalität der ersten Wahl ist die chirurgische Resektion. Empfohlen wird ein risikoadaptiertes Vorgehen im Sinne einer systematischen Randschnittkontrolle für Tumoren mit hohem Rezidivrisiko bzw. mit Risikolokalisation, um subklinische Tumorausläufer zu erfassen und gleichzeitig gesunde Haut zu schonen. Für weitere Therapieoptionen wie topische Medikation, destruktive Verfahren oder Radiatio gelten verschiedene Vorbehalte. In diesem Beitrag werden die Indikationen und Verfahrensweisen vorgestellt, wobei die chirurgischen Resektionstechniken den Schwerpunkt bilden.
