In Deutschland findet etwa ein Drittel aller Operationen ambulant statt, konkrete medikamentöse Konzepte sind hierbei bisher kaum untersucht. Wir behandelten 126 Patienten im Fachgebiet Orthopädie/Unfallchirurgie intra- und postoperativ mit Metamizol und befragten sie am ersten postoperativen Tag mit dem QUIPS-Patientenfragebogen. 79 Patienten (62,7 %) berichteten über postoperative Schmerzen von ≥4 (NRS) bei Bewegung. 84 (66,6 %) unserer Patienten gaben an, in ihrer Mobilität schmerzbedingt beeinträchtigt zu sein, 48 (38,1 %) hatten schmerzbedingte Schlafstörungen und 26 (20,6 %) hätten sich gewünscht, mehr Schmerzmittel zur Verfügung zu haben. Dennoch waren 91 (72,2 %) der Patienten mit der Schmerztherapie eher und sehr zufrieden. Möglicherweise wäre dem schwierig zu behandelnden, nozizeptiven Bewegungsschmerz durch Gabe eines NSAID oder Cox-2-Hemmers besser zu begegnen gewesen. Weitere Untersuchungen müssen folgen.
