Die Hyponatriämie ist die häufigste Elektrolytstörung in der Notaufnahme. Die Symptomatik ist unspezifisch, die Patienten zeigen Übelkeit, Schwindel und oft Stürze. Für schwere Hyponatriämien sind Erbrechen, zerebrale Krampfanfälle und Somnolenz bis hin zum Koma typisch. Die spezifische initiale Labordiagnostik umfasst die Messung der Serumelektrolyte, der Serumglukose, der Serum- und Urinosmolarität sowie des Urinnatriums. Die klinische Untersuchung hat die wesentliche Aufgabe, zu einer sicheren Einschätzung des Volumenstatus zu führen. Ist der Patient hypovoläm, ist die Gabe von NaCl 0,9 % das Mittel der Wahl. Wirkt der Patient euvoläm, kommen Ursachen wie das Syndrom der inadäquaten Sekretion von antidiuretischem Hormon (SIADH) oder (neurotrope) Medikamente als Ursache infrage. In diesen Fällen ist die Flüssigkeitsrestriktion die primäre Maßnahme. Da eine zu schnelle Normalisierung zur pontinen Myelinolyse führen kann, muss der Anstieg der Serumkonzentration in den ersten 24 h < 10 mmol/l und in den ersten 48 h < 18 mmol/l betragen. Die klinische Symptomatik einer Hyperkaliämie umfasst neurologische Symptome (Schwäche der Muskulatur bis hin zur Lähmung, aber auch Hyperreflexie und Krämpfe sowie Parästhesien), gastrointestinale Beschwerden (Übelkeit, Erbrechen, Durchfälle) und v. a. kardiale Symptome (Rhythmus- und Überleitungsstörungen). Therapeutisch wirkt eine Kalziumgabe sofort. Zur raschen Kaliumsenkung durch Verschiebung von Kalium in intrazellulärer Richtung sind die Gabe von Glukose mit Insulin und die hoch dosierte inhalative Applikation von β-Mimetika geeignet. Durch Infusion von NaCl 0,9 %, i.v.-Gabe von Furosemid, Einsatz von Ionenaustauscherharzen und Hämodialyse ist die Elimination von Kalium möglich.
