Hintergrund
Kürzlich wurde von der mittleren Wirkdosis (ED50) für die motorische Blockade von einfachem spinalem Bupivacain bei Erwachsenen berichtet, die von Ropivacain ist jedoch unbekannt. Diese Studie hat zum Ziel, die ED50 für die motorische Blockade von einfachem spinalem Ropivacain bei Erwachsenen zu bestimmen und den Einfluss des Alters auf diesen Parameter zu bewerten.
Methoden
133 Patienten, die kombinierter Spinal- und Epiduralanästhesie bedurften, wurden in die sequentielle Allokationsstudie aufgenommen. Sie wurden nach Alter in sechs Gruppen aufgeteilt: 20–30, 31–40, 41–50, 51–60, 61–70 und 71–80 Jahre. Unter Anwendung der Up-and-Down-Methode von Dixon wurde die Ropivacain-Dosis je nach vorangegangener Reaktion des Patienten variiert. Anhand der modifizierten Bromage-Skala und der Hüftmotorik wurde das Ausmaß der motorischen Blockierung nach der Ropivacain-Verabreichung bewertet. Die ED50-Werte wurden nach der Methode von Dixon und Massey geschätzt. Andere Messziffern, darunter das Einsetzen der motorischen Blockade, die Analgesiestärke und die Dauer der motorischen Blockade wurden ebenfalls erhoben.
Ergebnisse
Die ED50 für die motorische Blockade von intrathekalem Ropivacain lag bei 20,96 mg (95 % CI: 19,83–22,16 mg) für die Altersgruppe 20–30, bei 19,05 mg (95 % CI: 18,43–19,70 mg) für 31–40, bei 17,91 mg (95 % CI: 17,10–18,76 mg) für 41–50, 17,91 mg (95 % CI: 16,49–19,44 mg) für 51–60, bei 16,11 mg (95 % CI: 14,50–17,90 mg) für 61–70 und bei 15,75 mg (95 % CI: 13,98–17,73 mg) für 71–80 Jahre alte Patienten. Maximale kephalisch-analgetische Wirkungen wurden in allen Gruppen mit den L4–T6- und den L4–T2-Leveln bei jeweils 5 und 10 Minuten nach intrathekaler Ropivacain-Gabe erreicht.
Schlussfolgerung
Die ED50 für motorische Blockade bei einfachem spinalem Ropivacain nimmt mit zunehmendem Alter ab, was darauf hinweist, dass Alter einen Einfluss auf die Wirkstärke von spinalem Ropivacain hat.
