Hintergrund
Die Kyphoplastie ist ein hervorragendes Verfahren zur Behandlung osteoporotischer Sinterungsfrakturen der Wirbelsäule. Sie führt zur Aufrichtung der Kyphose und zur signifikanten Schmerzreduktion. Der Sachkostenanteil ist mit 53% aber sehr hoch. Ist die Kyphoplastie so kostendeckend möglich?
Methode
Anhand einer Prozesskostenanalyse wurden die Verläufe vom Erstkontakt eines Patienten bis zur Entlassung dokumentiert, Personen- und Zeitaufwendungen exakt gemessen und sämtliche anfallenden Kosten berechnet.
Ergebnisse
Bei 118 Patienten wurden 2009 Kyphoplastien einer Etage durchgeführt. Die mittlere Verweildauer betrug 6 Tage. Pro Patient wurden 6198,77 EUR erlöst. Bei Materialkosten von 3134,99 EUR (53%) fielen Personalkosten von 1552,86 EUR (26%) an. Die Personalkosten setzten sich aus dem ärztlichen Dienst (8 h 52 min = 551,93 EUR), der Pflege (20 h 9 min = 629,69 EUR) und dem medizinisch technischen Dienst (11 h 15 min = 371,24 EUR) zusammen. Die Gesamtkosten betrugen bei unseren Patienten 5868,23 EUR. Bei dem DRG-Erlös von 6189,77 EUR verblieben in unserem Krankenhaus als Erlös 321,54 EUR/Patient. Die Kyphoplastie war kostendeckend möglich.
