Operationsziel Verlangsamtes Fortschreiten einer medialen Gonarthrose durch Verlagerung der Belastungsachse vom medialen in das laterale femorotibiale Kompartiment und Reduktion der vermehrten Druckbelastung auf der erkrankten medialen Seite.
Indikationen Isolierte mediale Gonarthrose im Frühstadium.
Symptomatische Varusfehlstellung.
Entlastende Korrektur der mechanischen Beinachse bei knorpelrekonstruktiven Eingriffen wie der autogenen Chondrozytentransplantation.
Korrektur posttraumatischer Varusfehlstellungen.
Kontraindikationen Begleitende patellofemorale Arthrose, laterale Gonarthrose oder andere schmerzhafte Zustände des Kniegelenks.
Eingeschränkter Bewegungsumfang.
Knieinstabilitäten, da eine Progredienz zur Pangonarthrose wahrscheinlich ist.
Schlechter Schraubenhalt bei ausgeprägter Osteoporose.
Schlechte periphere Durchblutung mit fehlenden Fußpulsen.
Zustand nach lateraler Meniskektomie.
Operationstechnik Lateraler Zugang. Subkapitale Osteotomie der Fibula. Darstellen des lateralen Tibiakopfs. Partielle Osteotomie des proximalen Drittels der Tuberositas tibiae. Markieren der Gelenklinie. Einschlagen einer Halbrohrplatte über einen Führungsdraht parallel zur Gelenklinie, 1,5 cm distal davon. Aufsteigende Osteotomie des Tibiakopfs von kaudal-lateral nach kranial-medial. Die Osteotomie wird 2,5 cm distal der Platte angesetzt und sollte medial unmittelbar unter dem Führungsdraht enden. Dabei soll die mediale Kortikalis intakt bleiben. Entnahme eines lateral basierten Knochenkeils gemäß Planung. Schließen der Osteotomie unter dosiertem Valgusstress. Über eine durch das vorstehende Plattenloch geführte 4,5-mm-Kortikalis-Zugschraube wird die Osteotomie unter Kompression gebracht.
Weiterbehandlung Während der Hospitalisation regelmäßige Behandlung auf der passiven Bewegungsschiene ohne Bewegungseinschränkung nach Maßgabe der Beschwerden und Beginn mit isometrischen Kraftübungen. Mobilisation unter Teilbelastung mit 15 kg an zwei Unterarmgehstöcken während 6 Wochen postoperativ sowie ambulante Physiotherapie mit aktiven und passiven Bewegungsübungen. Danach radiologische Stellungs- und Konsolidierungskontrolle und je nach Befund schrittweiser Übergang zur Vollbelastung bis zur 10. postoperativen Woche. Thromboembolieprophylaxe mit niedermolekularem Heparin bis zum Erreichen der Vollbelastung.
Ergebnisse Die eigenen, nach der hier beschriebenen Technik operierten Patienten wurden retrospektiv analysiert. Anhand von Kaplan-Meier-Überlebenskurven konnte gezeigt werden, dass ein direkter Zusammenhang zwischen dem postoperativen Aktivitätsgrad der Patienten und der Langzeitüberlebensrate nach 10–15 Jahren besteht. Außerdem hat das Ausmaß der Achsenkorrektur einen direkten Einfluss auf das Langzeitresultat.
