Hintergrund Die zeitnahe Erkennung einer intrakraniellen Blutung hängt unmittelbar mit deren Prognose zusammen. Um bereits im präklinischen Bereich verlässliche Aussagen hinsichtlich des Bestehens einer intrakraniellen Blutung zu erhalten, wurde ein laseroptisches System entwickelt, welches bei optimalen Messbedingungen (u. a. kooperativer Patient, Fehlen von massiven Weichteilverletzungen) schnell den Verdacht auf eine kalottennahe Blutung bestätigen kann.
Methode Wir haben dieses System (CRAINSCAN, Fa. odicrain) bei 75 Fällen mit Blutungsverdacht eingesetzt. Die Ergebnisse wurden mit zeitnah angefertigten CT- bzw. MRT-Aufnahmen verglichen.
Resultate Bei 46 Patienten mit intrakranieller Blutung konnte das Gerät dieses in 30 Fällen richtig postiv anzeigen (65,2%). 29 Patienten ohne Blutnachweis in der Bildgebung wurden in 25 Fällen richtig negativ erkannt (86,2%).
Diskussion Relativ sicher können unilaterale kalottennahe Hämatome erkannt werden, wenn optimale Messbedingungen für das Gerät vorliegen—dies ist beim schweren SHT die Ausnahme. Die verlässlichsten Messungen gelangen bei Patienten mit unilateralen Hämatomen ohne schweres SHT. Nicht angezeigte Hämatome lagen häufig entweder zu kalottenfern bzw. zu mittelliniennah oder waren symmetrisch konfiguriert.
Unserer Meinung nach eignet sich das Gerät wegen vieler Störfaktoren und Einsatzbeschränkungen nicht für den Gebrauch beim schweren SHT sondern eher als zusätzliches diagnostisches Mittel bzw. zur klinischen Verlaufskontrolle bei mildem SHT ohne größere Weichteilverletzungen. Auch zur Differenzierung zwischen Hirninfarkt und spontanen intrazerebralen Hämatomen kann das System sinnvoll verwendet werden.
