Heute darf davon ausgegangen werden, dass fr die Diagnostik einer Venenkrankheit schon die Duplexsonographie zum Einsatz gekommen ist. Von der Phlebographie mssen deshalb darber hinaus gehende Informationen erwartet werden, die nur Rntgenbilder mit einer exzellenten Technik zu liefern vermgen. In der Phlebographie neu hinzugekommen ist der Darstellungsmodus des Venensystems in 3Phasen, der Leerphase, der Fllungsphase und der Auswaschphase. Dadurch gelingt auch die Beurteilung der Wadenmuskelvenen mit einer bisher nicht erreichten diagnostischen Genauigkeit. Bei der Phlebothrombose liegen spezielle Indikationen zur Phlebographie vor, um die verschiedenen Verlaufsformen gegeneinander anzugrenzen. Insbesondere geht es um die Erkennung der deszendierenden Thrombose. In Verbindung mit der Duplexsonographie ergeben sich daraus die Indikationen zur operativen Intervention. Fr die Begutachtung des postthrombotischen Syndroms hat die Phlebographie als Ergnzung der Duplexsonographie weiterhin eine groe Bedeutung. Bei der Indikationsstellung zur Venenklappentransplantation kann auch auf die retrograde Pressphlebographie nicht verzichtet werden. Bei der Operation der Stamm- und Perforansvarikose hat die Operation nach dem Rntgenbild im Prinzip nichts an Bedeutung verloren. Sie vermittelt dem Chirurgen eine groe Sicherheit bei der Planung des Eingriffs und strkt das Vertrauen des Patienten in die vorgeschlagenen Manahmen. Auerdem ist die sekundre Leitvenen-Insuffizienz bisher immer noch am besten durch die Phlebographie zu diagnostizieren. Die Phlebographie ist aus dem Katalog der bildgebenden diagnostischen Verfahren nicht herausgestrichen, sie ist nur weiter hinten an eine andere Stelle gerckt. Die vorderen Seiten des Katalogs werden heute ganz durch die Duplexsonographie belegt.
It can be assumed in this day and age that duplex sonography has been employed for the diagnosis of venous disease. Thus, we expect phlebography to provide additional information that can only be rendered by high-quality radiographs. The three-phase display mode is a new technique in phlebography consisting of an empty phase, filling phase, and rinsing phase. This enables assessment of the calf muscle veins with a diagnostic accuracy not feasible until now. There are special indications for phlebography in cases of phlebothrombosis to differentiate the different disease courses, in particular identification of descending thrombosis. Together with duplex sonography this contributes to the indication for operative intervention. Phlebography continues to serve as an important adjunct to duplex sonography for evaluation of the postthrombotic syndrome. The decision for venous valve transplantation cannot be made without retrograde press-phlebography. Surgery for trunk varicose veins and perforating varicose veins based on radiographs has not lost significance in principle. It imparts a sense of considerable confidence to the surgeon in planning the operation and reinforces the patient's trust in the measures recommended. Furthermore, secondary venous insufficiency is still best diagnosed by phlebography. Phlebography has not been banished from the ranks of diagnostic imaging techniques, but has be relegated to a different role. Duplex sonography has taken over the front line.
