In Zwillingsstudien wird die Erblichkeit relativ einheitlich auf etwa 0,60,7 beziffert; bei Familienstudien wurden teilweise hnlich hohe Erblichkeitsschtzungen erzielt. Auf DNA-Ebene ist es gelungen, bei etwa 2,5% aller extrem bergewichtigen Kinder und Jugendlichen Mutationen im Melanokortin-4-Rezeptor-Gen zu identifizieren. Diese Form der Adipositas wird dominant vererbt. Bei Erwachsenen fhren diese Mutationen zu einem Krpergewicht, das etwa 1530kg hher liegt als bei Familienmitgliedern mit Wildtypgenotyp; es handelt sich demnach um einen Hauptgeneffekt. Erste Fortschritte bei der Identifikation von Polygenen sind gemacht worden; entsprechende Genvarianten haben jeweils nur einen kleinen Effekt auf das Gewicht.
Twin, adoption, and family studies point to the relevance of genetic factors in body weight regulation. Relatively uniform heritability estimates in the magnitude of 0.6-0.7 have been obtained in twin studies; some family studies have come up with similarly high estimates. In molecular genetic terms the clinically most relevant finding pertains to mutations in the melanocortin-4 receptor gene (MC4R). Approximately 2.5% of extremely obese German children and adolescents harbor such mutations. This type of obesity is inherited in an autosomal dominant fashion. Adult mutation carriers are 1530 kg heavier than their family members with the wild-type genotype. Hence, functionally relevant MC4R mutations exert a major effect on body weight. Recently, single polygenes have been identified; the respective alleles have a minor effect on body weight.
