Hintergrund
Der Kopfschmerz gehört zur Lebenserfahrung der meisten Menschen und erlangt so seine Bedeutung als Volkskrankheit. Als komplexes, multidimensionales Konstrukt beruht die Klassifikation des Kopfschmerzes auf subjektiven Angaben Betroffener, die durch die Häufigkeit des Schmerzes, seine unterschiedlichen Qualitäten und ihre Wahrnehmung beeinflusst werden. In der Klassifikation des Schmerzes sind deshalb verschiedene methodische Probleme zu berücksichtigen, die die Schätzungen der Prävalenz oder Inzidenz einzelner Kopfschmerzerkrankungen beeinflussen. Ziel der vorliegenden Arbeit ist es, methodische Probleme bei der Erhebung und Klassifikation von Kopfschmerzen insgesamt sowie bei einzelnen Kopfschmerzerkrankungen aufzuzeigen und ihre Häufigkeit in Studien in der deutschen erwachsenen Bevölkerung zu beschreiben.
Methodik
Querschnitt- und prospektive Studien, die Kopfschmerzen insgesamt oder einzelne Kopfschmerzerkrankungen in Deutschland erhoben haben, werden dargestellt und die methodischen Probleme der Erhebung diskutiert.
Ergebnisse
Die Wahl unterschiedlicher Methoden, vor allem in der Klassifikation der häufigsten, primären Kopfschmerzerkrankungen, Migräne und Spannungskopfschmerz, führen dazu, dass die vorliegenden Studien nur wenig miteinander vergleichbar sind. Insgesamt besteht ein deutlicher Mangel an prospektiven Kohortenstudien, die eine Klassifikation einzelner Kopfschmerztypen erlaubt.
Schlussfolgerungen
Angesichts des Status als „Volkskrankheit“ liegen in Deutschland nur wenige Studienergebnisse zu Häufigkeit, Risikofaktoren, individuellen und ökonomischen Auswirkungen des Kopfschmerzes, basierend auf Primärdaten, vor. Methodische Probleme in der Erhebung und Klassifikation einzelner Kopfschmerzerkrankungen haben zu dieser Situation beigetragen. Eine weitere Standardisierung der Methoden in zukünftigen Studien ist wünschenswert.
