Hintergrund.. Bisher existieren keine Literaturangaben zu einer durch akute Pneumonie erforderlichen nichtinvasiven nasalen Maskenbeatmung bei McCune-Albright-Syndrom. Kasuistik.. Bei dem Patienten war im 3.Lebensmonat ein McCune-Albright-Syndrom diagnostiziert worden. Im Verlauf kam es zu multiplen Knochenfrakturen aller Extremitten. In den letzten Jahren traten rezidivierende Infekte und Bronchitiden auf, seit einigen Monaten bestand morgendlicher Frontalkopfschmerz. Der 16-jhrige Junge wurde aufgrund einer akuten respiratorischen Insuffizienz (pH 7,27, pCO2 8,96kPa, SpO2 92% trotz 6l/min O2-Gabe) aufgenommen. Im Rntgenthorax zeigte sich eine Unterlappenpneumonie links. In der Sputummikrobiologie wurde Streptococcus pneumoniae nachgewiesen. Die Spirometrie zeigte schwerste restriktive Ventilationsstrungen auch nach Abklingen der Pneumonie (inspiratorische Vitalkapazitt nur 200ml, entsprechend etwa 10% des Sollwerts). In der Pulsoxymetrie wurden rezidivierende kurzzeitige morgendliche Entsttigungen bis 60% trotz 3l O2-Supplementation gefunden. Der Patient tolerierte die nichtinvasive nasale Maskenbeatmung im kontrollierten Modus gut, die morgendlichen Kopfschmerzen sistierten unter dieser Therapie. Initial wurde ber 24h/Tag, im Verlauf nur nachts und zur Mittagsruhe mit 0,5lO2/min beatmet. Unter Maskenbeatmung, antibiotischer Therapie und Physiotherapie normalisierten sich die Blutgase, die pulsoxymetrisch gemessene Sauerstoffsttigung sowie die Entzndungsparameter. Die inspiratorische Vitalkapazitt stieg auf 370ml. Diskussion.. Bei Patienten mit akuter respiratorischer Insuffizienz infolge restriktiver Ventilationsstrungen auf dem Boden einer Atempumpschwche sollte an die Mglichkeit der nichtinvasiven Maskenbeatmung gedacht werden, um eine Intubation mit Intensivstationsaufenthalt und eine sptere Tracheotomie vermeiden zu knnen.

Sputum-microbiology: Streptococcus pneumoniae. Spirometry: most severe restrictive ventilatory disorder also after pneumonia went down (inspiratory vital capacity 200ml, about 10% of rated value). Pulsoximetry: recurrent short O2-desaturation in the morning till 60% despite 3l O2-supplementation. Course and therapy: well tolerated non-invasive nasal mask ventilation in controlled mode with stop of the frontal headache. Initial ventilation with a duration of 24h/d, later only at night and at afternoon nap with 0,5l O2/min. During mask ventilation, antibiotical therapy and physiotherapy normalisation of the blood gas value, O2-saturation (measured by pulsoximetry), inflammatory parameters and slightly risen inspiratory vital capacity up to 370ml. Discussion.. In patients with acute respiratory failure due to restrictive ventilatory disorder because of weakness of breath-pump the possibility of non-invasive mask ventilation should be considered, to avoid Intubation on intensive care units and tracheotomy.
