Fragestellung und Hintergrund: Es war Ziel dieser Studie, die Inzidenz von defäkographisch nachgewiesenen Sigmoidzelen bei Patienten mit Entleerungsstörung (Defäkationsobstruktion) bei klinisch manifester Intussuszeption (innerer Rektumprolaps) und ventraler Rektozele zu evaluieren.
Methodik: In die Studie eingeschlossen wurden nur diejenigen Patienten, die unter einer Entleerungsstörung litten und bei denen im Rahmen der proktologischen Untersuchung mit Proktorektoskopie ein innerer Rektumprolaps (Mukosaprolaps oder rektorektale bzw. rektoanale Intussuszeption) und eine ventrale Rektozele nachgewiesen worden waren. Die Daten von 297 Patienten, die innerhalb eines 6-Jahres-Zeitraums (1999–2005) im Rahmen der Proktologischen Sprechstunde eine Beckenbodenfunktionsdiagnostik erhielten, wurden retrospektiv sowohl auf die bildmorphologische Relevanz der Intussuszeption und Rektozele als auch auf die Koinzidenz einer Sigmoidozele und asoziierter Funktionsstörungen des Beckenbodens evaluiert. Hierzu wurden die Defäkographien (n = 210) bzw. dynamischen Beckenboden-MRT (n = 87) ausgewertet. Bei allen Patienten erfolgte vor der bildmorphologischen Funktionsdiagnostik eine standardisierte Anamnese (inklusive Obstipationsscore) mit klinischproktologischer Untersuchung.
Ergebnisse: Im Beobachtungszeitraum wurden die Daten von 297 Patienten (274 Frauen, 23 Männer; mittleres Alter 57 Jahre, mittlerer BMI 24,3 kg/m2) ausgewertet. Alle Patienten hatten bei der klinischen Untersuchung einen inneren Rektumprolaps (ventraler Mukosaprolaps, rektorektale oder rektoanale Intussuszeption) mit ventraler Rektozele. Im Rahmen der Funktionsdiagnostik (210 Defäkographien, 87 dynamische Beckenboden-MRTs) zeigten sich bei 46,5% (n = 138) bzw. 23,9% (n = 71) bildmorphologisch relevante Rektozelen (> 2 cm) bzw. die Entleerung behindernde Intussuszeptionen. Ein Descensus perinei (> 3 cm) wurde bildmorphologisch bei 34,0% (n = 101) festgestellt. Von den 297 Patienten wiesen defäkographisch bzw. MRT-morphologisch 109 Patienten (36,7%) eine Sigmoidozele auf (Sigmoidozele I. Grades: n = 71; Sigmoidozele II. Grades: n = 7; Sigmoidozele III. Grades: n = 31).
Schlussfolgerung: Die klinische und therapeutische Relevanz bei Patienten mit Entleerungsstörung bei klinisch nachgewiesenem inneren Rektumprolaps und ventraler Rektozele ist kontrovers, weil vergesellschaftete Störungen häufig sind. Die eigenen Ergebnisse der bildgebenden Diagnostik belegen, dass bei einem Drittel der Patienten mit innerem Rektumprolaps eine Sigmoidozele vorliegt (36,7%). Zudem zeigt die Defäkographie nur bei einem Viertel eine bildmorphologische Relevanz des inneren Rektumprolaps (23,9%). Inwieweit aus diesen Befunden eine funktionelle Relevanz mit entsprechenden therapeutischen Konsequenzen abgeleitet werden kann, bleibt eine klinische Herausforderung.
