Strken sind die hohe intraoperative Sicherheit,Rotationssicherheit bei stabilen 31.A1-Frakturen und Dynamik im Frakturbereich, die eine Knochenbruchheilung unter Belastung begnstigt.Schwchen sind: fehlende Absttzung des instabilen Kopf-Hals-Fragments nach lateral mit der Folge einer Medialisierung des Schafts und ungengende Rotationssicherung bei den instabilen 31.A2- und 31.A3-Frakturen. Fehlbohrungen bei ungengender Lage des zentralen Fhrungsdrahts knnen aufgrund des groen Bohrdefekts verheerende Folgen haben. Die Ergnzung des Implantats mit einer Trochanterstabilisierungsplatte (TSP) fhrte zu einem modularen System. Konkurrenz fr das Produkt entstand mit der Entwicklung eines geschlossenen, intramedullren Verfahrens,welches als -Nagel Anfang der 90er Jahre auf den Markt kam. Die randomisierten Studien konnten bisher im Gegensatz dazu keine besseren Ergebnisse fr die intramedullren Verfahren beweisen. Die Auswertung aller randomisierten, prospektiven Studien ergab im Gegenteil bis einschlielich 1999 eine niedrigere Rate an Femurschaftfrakturen und eine niedrigere Rate an Reoperationen bei der Anwendung der DHS.Die verbesserten Versionen der intramedullren Verfahren werden den Beweis ihrer berlegenheit noch anhand von weiteren Studien erbringen mssen.

The advantages of this device are its high intraoperative safety, rotational stability in stable 31.A1 fractures, and the dynamics allowed in the fracture area, which leads to improved fracture healing with weightbearing. Over the years it has become apparent that it also has limitations: lacking lateral support for the unstable head-neck fragment leading to medialisation of the shaft and inadequate rotational safety in unstable 31.A2 and 31.A3-fractures.Faulty drilling with inadequate positioning of the guidewire can have deleterious effects owing to the resultant large drilling defect.The addition of a trochanteric stabilisation plate (TSP) to the implant has led to a modular system that can mitigate the weaknesses of the original version of the implant.Competition for this product emerged with the development of a closed intramedullary procedure using an implant that has been marketed as the gamma nail since the early 1990s. Nonetheless, evaluation of all randomised, prospective studies published up to 1999 inclusive has shown a lower rate of femur fractures and a lower rate of revision operation following use of the DHS.The improved versions of intramedullary devices will have to demonstrate their supposed superiority in further studies.
