Fragestellung. Ziel war die Implementierung eines Protokolls für die abdominelle CT-Angiographie an einem 16-Detektorzeilen-CT. Zudem erfolgte der Vergleich mit der selektiven Katheterangiographie.
Methoden. Bei 76 Patienten mit verschiedenen vaskulären Erkrankungen bzw. Fragestellungen erfolgte eine abdominelle CT-Angiographie an einem neuen 16-Detektorzeilen-CT-System mit Submillimeter-Schichtkollimation (16×0,75 mm). Bei 17 Patienten wurde die CT-Angiographie der A. mesenterica und der A. hepatica mit der selektiven Angiographie, die im Rahmen von Tumorinterventionen durchgeführt wurde, verglichen. Die Kontrastmittelapplikation wurde durch eine Testbolusmessung individuell angepasst. Zur Evaluierung des Kontrastmittelprotokolls wurden die Dichtecharakteristika der durchgeführten CT-Angiographie analysiert.
Ergebnisse. Bei allen Patienten mit Vergleichsangiographie (n = 17) konnte eine diagnostisch ausreichende Bildqualität erzielt werden. Im Stromgebiet der A. hepatica und der A. mesenterica superior konnten Seitenäste bis zur 4. Ordnung abgegrenzt werden. Im Vergleich zur selektiven Angiographie lieferte die CT-Angiographie bei allen Patienten die gleiche morphologische Aussagekraft. Die gewählte Kontrastmittelapplikation erlaubte eine homogene Kontrastierung der gesamten abdominellen Aorta und Iliakalstrombahn.
Schlussfolgerung. Mit 16-Detektorzeilen-MSCT können angiographische Darstellungen des gesamten Abdomens generiert werden. Die erzielbare räumliche Auflösung liegt in einer Atemanhaltephase im Submillimeterbereich, sodass 3D-Darstellungen in hoher Qualität rekonstruiert werden können. Der Vergleich mit der Katheterangiographie zeigt eine gute Übereinstimmung.
