Einleitung Ektope atriale Tachykardien (EAT) sind häufig refraktär gegenüber einer medikamentösen antiarrhythmischen Therapie. Die Hochfrequenzstrom-Ablation ist eine sichere alternative Therapieoption zur Behandlung dieser Herzrhythmusstörung. In der vorliegenden Studie werden unsere Ergebnisse der Ablationsbehandlung atrialer Ektopien mit einem neuen Mappingsystem (Stablemapr®, Medtronic) dargestellt.
Methoden Eingeschlossen in die Studie wurden 30 konsekutive Patienten mit nachgewiesener rechtsatrialer EAT. Bei 15 Patienten (G1) wurde die Lokalisationsdiagnostik mit dem 20-poligen Stablemapr® durchgeführt. Die Prozedurdaten wurden mit einer Kontrollgruppe (G2, n=15), bei der das Mapping konventionell erfolgte, verglichen. Die demographischen Charakteristika und die zugrundeliegenden strukturellen Herzerkrankungen waren zwischen den Gruppen vergleichbar. In der Gruppe 1 wurde die früheste lokale atriale Erregung der Tachykardie mithilfe des im rechten Atrium platzierten 20-poligen Mapping-Katheter lokalisiert. In der Gruppe 2 wurde der Fokus anhand von Punkt zu Punkt Messungen durch das Aufsuchen der frühesten endokardialen Erregung in Relation zu einer Referenzelektrode im hohen rechten Vorhof (Aktivierungsmapping) oder anhand der P-Wellen-Morphologie während atrialer Stimulation (Pacemapping) identifiziert.
Ergebnisse Alle Patienten konnten erfolgreich abladiert werden. Die Foki lagen in beiden Gruppen ähnlich verteilt (G1/G2): Einmündung Vena cava superior (3/5) und inferior (1/0), freie Wand (3/3), Koronarsinusostium (3/3) und interatriales Septum (5/4). Die mittlere Untersuchungszeit (G1: 88±33 vs. G2: 151±61 min; p=<0,05) und die Durchleuchtungszeit (G1: 19±9 vs. G2: 38±28 min; p=<0,05) waren in der Gruppe 1 signifikant kürzer. Die Anzahl erforderlicher Stromabgaben wurde durch das neue Mappingsystem signifikant reduziert (G1: 10±10 vs. G2: 16±13; p=<0,05).
Schlussfolgerungen Die Ablation rechtsatrialer ektoper Tachykardien ist unter Verwendung eines 20-poligen Mapping-Katheters sicher und erfolgreich durchführbar. Die größten Vorteile im Vergleich zu herkömmlichen Mappingstrategien liegen in der verkürzten Untersuchungs- und insbesondere Durchleuchtungszeit.
