Die fachliche Spezialisierung in der Medizin, ausgelöst durch den immensen Zuwachs von Fachwissen und getragen durch die Weiterentwicklung medizinischer Technologien, führte zur Abtrennung von Teilgebieten aus den Mutterfächern und einer Umorientierung hin zu organbezogenen Partnern. So finden sich operative und interventionelle Fachgebiete häufig in zuvor unbekannter Nähe wieder. Spannungen durch Überlappungen im Tätigkeitsfeld blieben nicht aus und haben in der Vergangenheit zu einem gemeinsamen Memorandum der Fachgesellschaften geführt. Die Anfänge dieser Entwicklungen liegen nun 10 Jahre zurück und haben vielerorts neue Strukturen hervor gebracht. Viszeralchirurgen und Gastroenterologen werden parallel in der interventionellen Endoskopie ausgebildet und bilden die Grundlage für die interdisziplinäre Zusammenarbeit in Notfallversorgung, Diagnostik, Therapie und beim Komplikationsmanagement sowie der Palliation. Technische Weiterentwicklungen zur Minimierung des operativen Traumas wie z. B. NOTES bedürfen der Expertise beider Fachdisziplinen, um diese Innovation flächendeckend zum Einsatz zu bringen. Treibende Kraft gemeinsamer Entwicklungen sollte dabei unser Streben nach ständiger Verbesserung der Patientenversorgung durch ein hohes Niveau individueller Expertise, patientenfreundlicher Prozessoptimierung und adäquater Ressourcennutzung sein.
