Résumé
Le diagnostic d’une maladie de système et/ou sa prise en charge en service de réanimation est conditionné par le dialogue entre le réanimateur et les spécialistes de ces pathologies. Néanmoins, certains signes d’alerte cliniques (associations syndromiques) ou biologiques doivent amener le réanimateur à la suspicion de ces pathologies au départ. Certains examens biologiques ou histologiques sont rapidement réalisables en réanimation pour permettre de confirmer le diagnostic. La thérapeutique est souvent menée dans le même temps, avec une gestion souvent simultanée de la poussée de la maladie systémique et d’une complication infectieuse qui nécessite elle-aussi une expertise microbiologique poussée. La suppléance des fonctions vitals en attendant l’effet des traitements spécifiques peut aller de la ventilation non invasive jusqu’à l’assistance respiratoire extra-corporelle. Les centres de compétence et/ou de référence sont là pour valider des options thérapeutiques parfois hors normes chez ses patients sévères. Une démarche intégrative diagnostique et thérapeutique est proposée pour guidée le réanimateur dans cette prise en charge.
