Hintergrund
Trotz immer effektiverer immunmodulatorischer Pharmaka kommt es bei der multiplen Sklerose und ihrer Manifestation am Sehnerv in Form der Optikusneuritis zu einer irreversiblen axonalen Schädigung. Eine vom Immunsystem unabhängige, neuroprotektive Therapie könnte daher von zusätzlichem Nutzen sein.
Ergebnisse und Diskussion
Wir haben Erythropoietin als bereits zugelassene und in hohem Maße zytoprotektive Substanz identifiziert, welche diesem therapeutischen Ziel dienen könnte. Nachdem eine Vielzahl ermutigender präklinischer Versuche zur Neuroprotektion des Sehnerven durch Erythropoietin durchgeführt wurde, zeigte die klinische Pilotstudie VISION PROTECT, dass 33.000 IE Erythropoietin, intravenös an jeweils 3 aufeinanderfolgenden Tagen gegeben, die Abnahme der retinalen Nervenfaserschichtdicke signifikant reduzieren kann. Diese Therapie soll nun in der multizentrischen, prospektiven, doppelt verblindeten klinische TONE-Studie (Treatment of Optic Neuritis with Erythropoietin, NCT01962571) evaluiert werden. Primäre Endpunkte sind dabei die Dicke der retinalen Nervenfaserschicht und die Niedrigkontrastsehschärfe.
