Resumen
Introducción: Se llevó a cabo un análisis farmacoeconómico, para comparar la eficiencia de dos tratamientos antibióticos en las infecciones complicadas de piel y tejidos blandos: tigeciclina y la combinación de vancomicina y aztreonam.
Métodos: Análisis retrospectivo, determinístico, de minimización de costes. La eficacia de los tratamientos se estimó a partir de los resultados combinados de dos ensayos clínicos, aleatorizados, doble ciego, en los que se compararon tigeciclina (100 mg inicialmente, seguidos de 50 mg IV cada 12 horas) y vancomicina- aztreonam (1.000/2.000 mg IV cada 12 horas) administrados hasta un máximo de 14 días (538 y 519 pacientes tratados con tigeciclina y vancomicinaaztreonam, respectivamente, en la población de análisis por intención de tratar modificada). La utilización de recursos se estimó a partir de los ensayos clínicos y de fuentes españolas y los costes unitarios mediante una base de datos de costes sanitarios españoles.
Resultados: Como los ensayos clínicos se diseñaron para demostrar equivalencia, no hubo diferencias significativas de eficacia entre los tratamientos, con una tasa de curación clínica del 79,7% y 81,9%, respectivamente (p = 0,4183) por lo que se hizo un análisis de minimización de costes. No obstante, con tigeciclina se redujo la estancia hospitalaria, tanto en planta como en UCI (— 1,2 días en ambos casos) con menores costes de administración IV (-60,12 €). En el caso básico el coste medio de la enfermedad por paciente fue de 3.732 € con tigeciclina y 3.995 € con vancomicina-aztreonam (diferencia de 263 €). El resultado fue estable en el análisis de sensibilidad con ahorros por paciente tratado con tigeciclina de 240-2.819 €, excepto con costes mínimos, con un coste adicional de 35 € por paciente.
Conclusiones: Un mayor coste de adquisición no implica necesariamente un mayor coste de la enfermedad. Por cada paciente con infección complicada de piel y tejidos blandos, tratado con tigeciclina, se podrían ahorrar 263 €, en comparación con vancomicina-aztreonam.
