Hintergrund
Ein besonderer Aspekt des psychiatrischen Blockpraktikums besteht darin, dass viele Studenten dem Fachgebiet instinktiv mit Zurückhaltung begegnen. Es stellt sich die Frage, auf welche Weise kontroverse Themen wie die Elektrokonvulsionstherapie (EKT) angesprochen werden können. Grundsätzlich ist das Praktikum prädestiniert, zukünftigen Ärzten die Grundzüge der Therapiemethode zu vermitteln. Als Multiplikatoren und potenzielle Zuweiser können sie mit diesem Wissen der Chronifizierung schwerer psychischer Erkrankungen entgegenwirken.
Methodik
Zu Beginn des psychiatrischen Praktikums wurden 158 Medizinstudenten der RWTH Aachen nach dem Zufallsprinzip in drei Gruppen eingeteilt. Die erste Gruppe nahm aktiv an einer EKT-Behandlung teil (EKT-Gruppe), der zweiten Gruppe wurde ein Lehrvideo zur EKT gezeigt (Videogruppe), die dritte Gruppe diente als Kontrollgruppe. Die Studenten wurden vor und nach dem Praktikum hinsichtlich ihres Wissens und ihrer Einstellung zur EKT befragt.
Ergebnisse
Im Verlauf des Praktikums nahmen die Studenten die EKT in allen Items positiver wahr. Bei den Teilnehmern der EKT- und der Videogruppe, nicht aber bei der Kontrollgruppe, wuchs die Bereitschaft, einer EKT-Behandlung bei Patienten, Angehörigen und Freunden und sich selbst zuzustimmen. Im direkten Vergleich der Interventionen war nur die EKT-Gruppe der Kontrollgruppe überlegen. Das Wissen über EKT nahm trotz der Begrenztheit der beiden Interventionen im Vergleich zur Kontrollgruppe deutlich zu.
Diskussion
Bedenken, das kontroverse Thema EKT im Blockpraktikum aufzugreifen, bestätigten sich nicht. Bereits eine einmalige praxisnahe Intervention war hilfreich, um das Wissen und die Einstellung zur EKT zu verbessern. Diese Möglichkeit könnte intensiver genutzt werden.
