Hypogonadismus beschreibt beim Mann eine Strung der endokrinen und/oder exokrinen Testesfunktion. Der primre wird vom zentralen Hypogonadismus unterschieden. Beim primren Hypogonadismus sind die Hoden unmittelbar geschdigt und die Gonadotropine werden ungebremst, verstrkt sezerniert (hypergonadotroper Hypogonadismus). Der zentrale Hypogonadismus wird durch hypothalamische oder hypophysre Strungen unterschiedlichster tiologie verursacht. GnRH bzw. LH und FSH werden vermindert gebildet (hypogonadotroper Hypogonadismus). Die hufigsten angeborenen Formen des zentralen Hypogonadismus sind der idiopathische hypogonadotrope Hypogonadismus (IHH) und das Kallmann-Syndrom; ein erworbener zentraler Hypogonadismus wird am hufigsten durch Hypophysenadenome verursacht. Die Klinik des zentralen Hypogonadismus wird bestimmt durch Ausma und Manifestationszeitpunkt des Testosteronmangels und die Strung der Spermatogenese. Wegen der langsamen Entwicklung der krperlichen Vernderungen kommt der Anamnese eine besondere Bedeutung zu. Die Diagnose wird durch die Bestimmung von Testosteron und Gonadotropinen gesichert, dabei ist Testosteron subnormal, LH und FSH subnormal bis normal. Aufgrund der vielfltigen Ursachen kann eine umfangreiche Diagnostik notwendig werden. Eine Therapie wird nach Diagnostik und ggf. Behandlung einer dem Hypogonadismus zugrunde liegenden Erkrankung eingeleitet. Bei Kinderwunsch besteht die Mglichkeit einer Fertilittsbehandlung mit hCG/hMG, bzw. der pulsatilen Gabe von GnRH. Besteht kein Kinderwunsch, wird Testosteron substituiert; hierbei finden intramuskulre, orale oder transdermaler Applikationsarten Verwendung.
Male hypogonadism signifies a disorder in endocrine and/or exocrine testicular function. Primary hypogonadism is distinguished from central hypogonadism. Primary hypogonadism occurs due to testicular failure, and the secretion of gonadotropins is enhanced (hypergonadotropic hypogonadism). Central hypogonadism is caused by hypothalamic or pituitary disorders of multifarious etiology. Production of gonadotropin releasing hormone (Gn-RH) or luteinizing hormone (LH) and follicle-stimulating hormone (FSH) is diminished. The most frequent forms of congenital central hypogonadism are idiopathic hypogonadotropic hypogonadism (IHH) and Kallmanns syndrome; acquired central hypogonadism is most frequently caused by pituitary adenomas. The clinical manifestation of central hypogonadism is determined by the degree of testosterone deficiency, the time of onset, and the impairment of spermatogenesis. Because of the gradual development of the physical changes, the patients history is of particular importance. The diagnosis of central hypogonadism is confirmed by measurement of testosterone, LH, and FSH. In general, subnormal testosterone levels are accompanied by subnormal or normal LH and FSH levels. In most cases, a thorough diagnostic approach is necessary. The therapy of central hypogonadism is delayed until a diagnosis of the underlying disease is made. The treatment of infertility is based upon the application of human chorionic gonadotropin/human menopausal gonadotropin (hCG/hMG) or the pulsatile application of Gn-RH. Lifelong substitution therapy with testosterone is carried out by intramuscular, oral, or transdermal application.
