Résumé Aim
Une étude a été réalisée sur des chevilles de cadavres dans le but d’évaluer la stabilité de deux différentes voies d’abord pour le vissage interfragmentaire dans les fractures de la malléole latérale (externe).
Matériel et methods
Dix chevilles provenant de cinq cadavres de femmes ont été utilisées. La fibula a été ostéotomisée distalement aur niveau de la syndesmose à l’aide d’une scie, puis la fixation fracturaire a été faite de façon différente dans deux groupes. Dans le Groupe I les fractures ont été fixées par un vissage cortical antéro-postérieur traditionnel alors que dans le Groupe II, la fracture de la cheville controlatérale a été fixée à l’aide d’un vissage cortical postéro-antérieur. La fibula distale dans les deux groupes a été soumise à des forces biomécaniques de compression et de torsion et la force à laquelle la fixation a cédé a été enregistrée.
Résultats
Dans le Groupe I la force nécessaire pour casser le montage a été trouvée significativement plus faible que dans le Groupe II, avec une moyenne de 0.4 kN.
Conclusion
La fixation du Groupe II a été trouvée la meilleure.
