Résumé Introduction
Les cancers du col sont des pathologies fréquentes dans les pays en développement. Plus de 60 % des cas sont localement avancés, considérés comme non carcinologiquement résécable à cause de l’envahissement fréquent des structures pelviennes.
Objectifs
Cette étude a pour objectif d’évaluer la résécabilité des cancers localement avancés du col utérin après radiothérapie externe néoadjuvante.
Patientes et méthode
L’étude était rétrospective et portait sur 48 cancers du col de stades IIA: 18,8 %, IIB: 58,3 %, III: 16,7 % et IVA: 6,2 % en réponse objective opérés après radiothérapie néoadjuvante de 2000 à 2003. L’âge moyen était de 48,8 ans et 95,8 % étaient des carcinomes épidermoïdes. Les paramètres étudiés étaient l’évaluation chirurgicale, le résidu tumoral macroscopique, le degré de stérilisation tumorale et le suivi.
Résultats
Le délai moyen de la chirurgie était de 13,2 semaines. Nous avons réalisé dans 75 % des cas une colpohystérectomie radicale de type III de Piver, 12,5 % de type I et 8,3 % de type II. Le curage ganglionnaire iliaque était pratiqué dans 26 cas. Les complications étaient: quinze cas d’hémorragies peropératoires, deux cas de plaies urétérovésicales, deux cas de fistules urinaires et un cas de cystite radique. Deux patientes étaient décédées d’embolie pulmonaire. Un résidu tumoral macroscopique était noté dans 29,2 %. L’évaluation histologique portée sur 30 cas avait montré 60%de stérilisation complète et 40%de stérilisation partielle. Le recul moyen était de 40 mois. Le taux de contrôle locorégional était de 60,4 %. À 36 mois, la survie globale était de 55 %, la survie sans résidu de 68 % vs 20 % et la survie sans récidive de 92 % vs 10 %.
Conclusion
Très peu de malades présentant les cancers localement avancés du col sont traités. Nos résultats sont à valider par des études cas témoins entre patientes traitées par radiothérapie exclusive et patientes traitées par radiothérapie suivie de chirurgie.
