Die Bedeutung der ortsstndigen Aktivitt der Arteriosklerose fr die Neointimaformation und die Prognose einer Gefrekonstruktion ist unklar. Anhand des klinischen Verlaufs nach halbgeschlossener, angioskopisch kontrollierter Ringdesobliteration der A.femoralis superficialis wurde modellhaft der Einfluss der autochthonen Zellproliferation zum Zeitpunkt des operativen Eingriffs auf die Reverschlussrate untersucht. Von 40Patienten standen arteriosklerotische Verschlusszylinder zur histomorphologischen und immunhistochemischen Untersuchung zur Verfgung. Zellulre Proliferation, erkennbar am Nachweis des Kernantigens Ki-67, fand sich konzentriert in Regionen reparativen Plaqueumbaus sowie in Organisationszonen von luminalen und muralen Thromben und Plaquehmorrhagien. In der retrospektiven Analyse waren immunhistochemische Parameter mit der Durchgngigkeit der Rekonstruktion im klinischen Verlauf nicht signifikant assoziiert. In der Media und damit im Resektionsrand fand sich jedoch eine signifikante Prvalenz proliferativer Aktivitt, die angesichts der inflammatorischen Konditionierung der desobliterierten Gefwand Beachtung verdient. Die Ergebnisse sind konsistent mit der Schlussfolgerung, dass das mit der arteriellen Desobliteration verbundene Gefwandtrauma im individuellen Fall zu einer Aktivierung subklinisch bereits vorhandener inflammatorischer, reparativer und damit proliferativer Prozesse in den verbleibenden Wandschichten fhrt. Vor dem Hintergrund vergleichender Befunde an der A.carotis werden die Ergebnisse in ihrer biologischen Wertigkeit fr die Prognose nach arterieller Desobliteration diskutiert (DFG Br 1583/12).
The significance of the local activity of arteriosclerotic processes for neointima formation and patency of arterial reconstructions is still undefined. We investigated the impact of autochthonous cellular proliferation at the time of surgery upon the risk of reocclusion in plaque cylinders harvested by angioscopically controlled semi-closed endarterectomy of the superficial femoral artery. Samples of 40 patients were analyzed by histomorphology and immunocytochemistry. Cellular proliferation, as documented by the detection of the nuclear antigen Ki-67, was concentrated in regions of plaque tissue repair and in areas of organization of mural hemorrhage or luminal clots. According to retrospective analysis, there was no significant correlation of immunocytochemical parameters with patency of the reconstruction during follow-up. However, current proliferative activity in the media including the resection margins was significantly prevalent, which is of interest with respect to the baseline atherosclerotic inflammation persisting in the residual vessel wall. The results are consistent with the view that the vascular trauma caused by endarterectomy individually may activate preexisting proliferative processes associated with inflammatory and reparative cellular activity. Against the background of comparative findings in the carotid artery, the results are discussed focusing on the prognostic value of immunocytochemical proliferation marking for restenosis formation following arterial disobliteration (supported by Deutsche Forschungsgemeinschaft, DFG Br 1583/12).
