AbstractAt least seven parameters have been described for the measurement of patellofemoralmalalignment on CT scanning; three of which measure lateral patellar tilt, two lateral patellar shift and two femoral trochlear dysplasia. We studied 22 knees in 18 patients complaining of patellofemoral pain in order to investigate the reproducibility of these methods. CT scans of the patellofemoral joint were performed in each knee at 0 and 20 of flexion. The seven parameters were recorded from each scan by three independent observers in a blind study. The reproducibility was studied by means of the intraclass correlation coefficient (ICC). Parameters measuring lateral patellar tiltshowed excellent reliability (ICC75%). The measurements of lateral patellar shift and femoral trochlear dysplasia showed a fair or poor correlation (ICC75%). We suggest that parameters for measuring lateral patellar tilt only should be used from CT scanning when planning treatment for patello-femoral malalignment.
RsumAu moins sept paramtres ont t dcrits pour mesurer le dsalignement fmoro-patellaire par le scanner. Trois dentre eux mesurent lnclinaison laterale de la rotule, deux le dplacement latral et les deux restant la dysplasie de la trochle fmorale. Pour connatre la fiabilit de ces mthodes de mesure on a tudi vingt deux genoux de dix huit sujets avec des douleurs fmoro- patellaire de plus de six mois devolution. On fit deux scanners de larticulation de chaque genou zro et vingt degrs de flchissement respectivement. Chaque scanner ft mesur en utilisant les sept paramtres, par trois observateurs independants, dans une tude aveugle. Deux mois aprs les mmes observateurs ont repet les mesures, nouveau par la technique aveugle. La reproductibilit des mesures a t etudi par le Coeficient de Correlation Intraclasse (CCI). On trouve une haute fiabilit pour les paramtres qui mesuraient linclinaison laterale de la rotule (ICC75%). Dautre part les differentes mesures du dplacement lateral et la dysplasie de la trochle fmorale ont demontr une reproductibilit modere ou mauvaise (ICC75%). On pense que seuls les paramtres mesurant linclinaison laterale de la rot-ule doivant tre pris en compte pour des dcisions thrapeutiques.
