Die Anorexia nervosa ist eine psychische Störung, die primär bei jungen Frauen auftritt. Besonders bei chronischen Krankheitsverläufen oder bei einem Body Mass Index <12 kg/m2 muss mit einer Mortalität von bis zu 20% gerechnet werden. Oft werden die akut vital gefährdeten Patientinnen in internistischen Intensivabteilungen aufgenommen und notfallmäßig behandelt. Sind die Patientinnen therapiebereit, können sie in psychiatrische/psychosomatisch-psychotherapeutische Kliniken verlegt und dort weiterbehandelt werden. Sind sie jedoch krankheitsbedingt nicht willens oder nicht in der Lage, weiter notwendige Behandlungsmaßnahmen zu akzeptieren, stellt sich die Frage, ob sie im Sinne des Betreuungsrechts noch “ihre eigenen Angelegenheiten” besorgen können. Nachdem uns mehrere vital gefährdete anorektische Patientinnen auf die geschlossene psychiatrische Station zuverlegt wurden, haben wir unter Nutzung der Möglichkeiten des Betreuungsrechts ein Therapiekonzept für diese Patientengruppe entwickelt. Primäres Ziel ist es, mit Hilfe einer kontinuierlichen Gewichtszunahme die vitale Gefährdung der Patientinnen zu beenden, um sie in eine Weiterbehandlung in Spezialkliniken für Essgestörte zu verlegen. Anhand der Therapieverläufe von 25 Patientinnen, die im Rahmen dieses Konzepts behandelt wurden, soll das von uns gewählte therapeutische Vorgehen dargestellt werden.
